Pro-GeneX Financial Assistance

Pro-GeneX Laboratories is committed to providing medically necessary laboratory services and offers a Financial Assistance Program to patients that have financial hardships or other extenuating circumstances.
What Will Pro-GeneX Financial Assistance Cover?
Pro-GeneX offers a Financial Assistance Program for laboratory services provided only in Pro-GeneX laboratories and applies to most laboratory services that a medical provider decides is needed. Financial assistance is determined on income and the amount of your medical bills. People eligible for financial assistance will not be charged more for medically necessary laboratory services than the amounts generally billed to insured people.

How Do I Apply?

  • Go to Pro-GeneX.com and download the financial assistance application.
  • If you received, or plan to receive, Pro-GeneX laboratory services, call (844) 794-3637 to request an email version of the financial assistance application.
  • If you are interested in payment options, please call (844) 794-3637.

Pro-GeneX Quick Pay Discounts for Uninsured Patients
Uninsured patients who do not qualify for other assistance programs (such as Medicaid) receive a discount on their bill when the patient pays the balance within 30 days of the date of service. Please call (844) 794-3637 to request a quick-pay discount and to make a payment.

How Do I Apply?
If you need help paying for laboratory services provided by Pro-GeneX, you should apply for financial assistance. Applications are available online at Pro-GeneX.com or you can request a copy of the application be sent to you via email or text message.

How to Qualify
After patients provide information about household income, household size, expenses, and other personal circumstances, a cost of care compatible with the patient’s ability to pay is determined.
Pro-GeneX laboratories uses an evaluative process that considers an individual’s family income and family size, and total amount of medical bills. The estimated ability to pay model attempts to determine what portion, if any, of an individual’s income may be available to go towards paying for medical debt. As the sliding scale increases, more of an individual’s income is potentially available to pay for medical services. In addition, individuals with catastrophic medical bills may qualify for assistance.

What to Include With Your Application
Please provide the following for all household members:

  • Your two most recent pay stubs or other proof of income from any source. If you are self-employed or unemployed, provide copies of your last three months of bank statements.
  • Your most recent federal tax return, including all forms included with your return.
  • Your current savings and checking account statements.
  • Medicaid denial letter (if applicable)
  • Any information about your financial situation you want considered.

If you cannot provide these, please explain why on the application form.

What Happens After I Apply?
A representative will review your information and determine if you qualify and communicate this to you. You may be asked to apply for Medicaid or other programs before you receive financial assistance from Pro-GeneX.

I Qualify, How Much Help Will I Receive?
The amount of financial assistance provided will be based on need. Pro-GeneX considers all information you provide with your application to determine this.
